Purpose of processing your data

As a healthcare provider, Go Physio processes your personal and sensitive data in order to provide you with this service

What information we collect about you

Go Physio will only collect the amount of personal information needed for the purpose for which you have sought treatment with us.

  • We collect personal data such as your name, date of birth, address, email address, telephone number.
  • In providing effective and appropriate assessment and treatment, we may require sensitive personal data, such as information that relates to medical conditions.

How we use the information about you

We have a legitimate interest in collecting the above data in order to facilitate and manage your assessment and treatments with Go Physio.

With your consent, we may then send you a Feedback Questionnaire so that we can monitor our performance. You may withdraw this consent at any time by any method convenient to you.

Data storage & transfer

As a healthcare provider, we have a legal obligation to retain your records for 8 years after your final treatment (or to age 25, if this is longer). On an annual basis we confidentially dispose of records in accordance with current legislation.

We store your data on secure servers and take all reasonable organisational and technical measures to prevent unauthorised access to any personal data you provide us.

  • Go Physio Ltd uses TM3 (Blue Zinc Ltd) to manage and store all patient data. TM3 is a cloud based ISO27001 accredited practice management software based in the UK. TM3 data is hosted on Google Cloud Platform using servers based in the UK and Europe (compliant with the EU-US Data Privacy Framework and the Swiss-US Data Privacy Framework)
  • All bookings and patient data is processed by our staff in the UK however for the purposes of company IT hosting and maintenance, some of this information is backed up on US-based servers that are compliant with the EU-U.S. Data Privacy Framework (DPF).
  • Physical (paper) records are stored in locked filing cabinets within secure premises.

Third Parties

  • We will not sell or rent your information to third parties.
  • We will not share your information with third parties for marketing purposes.

Disclose

All our staff has a legal duty to keep your information confidential. With your written consent, your personal data may be shared with:

  • An intermediate company instructing us on your behalf, to provide you with an assessment or treatment service.
  • Your GP or consultant, if directly relevant to or after your treatment with us.
  • Your health insurance company (if applicable).

Access to your information and correction

We want to make sure that your personal data is accurate and up to date. If at any time you believe that the information we hold and process about you is incorrect and/or not being used for its original purpose you can request to see this information and have it corrected or removed (subject to permitted exemptions).
